P004100 – 02 Sensor signals swapped ( Bank 1 sensor 2/ Bank 2 sensor 2 )

P004100 – O2 Sensor Signals Swapped (Bank 1 Sensor 2 / Bank 2 Sensor 2)

🔧 DTC P0041 – o²  sensor signals swapped ( B1S2 & B2S2)

Meaning:
The ECM has detected that the post-catalyst oxygen sensor connectors (Bank 1 Sensor 2 and Bank 2 Sensor 2) are interchanged/swapped.
These are the downstream O₂ sensors located after the catalytic converter.

🔍 Common causes

1. O₂ sensor connectors swapped after exhaust or transmission work.

2. Incorrect O₂ sensor installed (wrong bank).

3. Damaged or repaired wiring harness incorrectly routed.

4. Aftermarket sensors with mismatched connector colors.

5. ECM identifying reversed signal voltage patterns.

🚗 Symptoms 

Check Engine Light ON

Fuel trim issues (sometimes)

No major driveability complaints normally

Incorrect catalyst monitoring readings

🛠 How to diagnose ( step-by-step for workshop)

1. Identify Bank & Sensor Positions

Bank 1 → Side of cylinder 1

Bank 2 → Opposite side

Sensor 2 → Downstream (after catalytic converter)

Ensure your technician knows exact engine layout.

2. Check O₂ Sensor Connectors

This is the most common cause.

Lift vehicle and inspect B1S2 and B2S2 connectors

See if someone recently worked on exhaust / gearbox

Try to follow wiring harness from sensor to connector

If connectors are same color/shape, they can easily be mixed

Quick Test

Disconnect both downstream O₂ sensors → identify:

Wire color codes

Length of harness

Pin positions (often different)

3. Swap Connectors Temporarily (Test Method)

If you’re not sure:

1. Swap B1S2 connector with B2S2

2. Clear codes

3. Run engine for 5–10 minutes

4. Check live data:

Expected Behavior

If swapped correctly, voltage graphs should show correct signal location

If still wrong → issue is wiring or wrong sensor installed

4. Check Live Data on Scanner

Monitor:

O2B1S2

O2B2S2

Downstream O₂ signals should be:

Stable

Low activity (~0.6–0.8V steady after warm-up)

If the ECM shows:

B1S2 fluctuating like B2S2 → connectors/wires swapped

B2S2 showing B1S2 pattern → same issue

5. Verify Correct Sensor Installed

Some cars have different part numbers for:

Bank 1 sensor 2

Bank 2 sensor 2

Check:

Part number

Sensor wire length

Connector shape

6. Inspect Wiring Harness

Look for:

Previous repairs

Harness pulled too tight

Wrong routing

Melted wires near exhaust

🔧 Repair procedure 

✔ Swap connectors to correct bank
✔ Re-install correct sensors if wrong part used
✔ Fix any wiring routing issues
✔ Clear DTC and perform a drive cycle

This code never appears unless someone worked on the vehicle recently (exhaust, cat, transmission, engine mount, O₂ sensors).
